Renew an Expired License

If your license has been expired for less than two years, you must sbumit the following items in order to renew your license:

• License renewal application form;
• Pay renewal plus $50.00 late fee;
• Required CEUs.

If your license has been expired for more than two years, you must reapply for licensure as if you are an initial licensure candidate.
