Request List of Licensees
To request a list or roster of all license holders, please submit the List Request form by either e-mail or mail to the address listed on the form.
The information provided on the roster will include the names, business addresses, license numbers. We do not provide social security numbers, birthdates, phone numbers, home addresses, or email addresses.
There is no fee for an electronic list, however a paper list costs $0.25 per page.
